In-network with Independence Blue Cross and Amerihealth

*co-pays are accepted via cash or check only. Please be prepared at the time of service.

For all out of network patients, I will provide you with a superbill to submit to your insurance company for reimbursement. 

Payment is required at time of service via cash or check. 

Evaluations are $220 and subsequent treatment sessions are $175.
